Energize Shropshire, Telford & Wrekin, the County Sports Partnership and Partners have identified that there is a lack of provision of Sport and Physical Activity for people with disabilities within the area.  In order to try to address this they need to understand some of the barriers preventing participation and would appreciate it if you could complete the following questionnaire.  It should take no more than 15 minutes.
